Configuration appears in white screen

I have Simple-Review v2.0.7 and the configuration screen looks like this:

Looks like something is missing here! Yeap, the Confirm button. What can be wrong?

In Joomla turn on 'Debug System' and set 'Error Reporting' to maximum.

Confirm that you uploaded config.simple_review.php and all config.php files.
